Is it possible to get a fitting the increases the width of the shoulder strap? There is quite a bite when carry weight.

Kathmandu does not sell or manufacture any kind of fitting that you can attach to these straps for a wider fit. You use the adjustments at the top of the straps and the bottom to get the best fit. I recommend popping into your local Kathmandu store with the pack fully packed and the team in store will be able to help you fit this correctly to your body so it is not digging in.
Alternatively, you can take this pack to your local repair agent and see if they have a suggestion to add width to these straps or extra padding to make this the most comfortable fit for you.

How do you adjust the straps and what is the best height of the pack against the back? I've lent mine to people and it's not sitting right now

What is the range of back length on this bag?

The standard back length for this pack is around 45 cm, there is then an adjustment available to extend the back length by around 10 cm. The shoulders adjust to increase the length for taller people and the sternum strap moves up and down to sit comfortably at any length.

Great pack. Comfortable and durable.Super pumped on this pack. I use it for mountaineering and ski-touring. Good quality outer material feels like it can handle abuse like the rest of the XT range.
Still comfortable under heavy loads also!

Light n' strongThe pack can hold as much weight as you can fit in it, it's comfortable on the shoulders and has decent storage space. The only downside is that it is a bit hard to compartmentalise as there is only one significant inner compartment so things at the bottom take a little longer to get out.

Very functional and quality bagI recently took this bag on it's first trip to Banff, AB. It was the perfect size for all of my hiking and camping gear and even has straps all around the outside for if I needed to strap on any additional camping pads or gear etc.
It is a very comfortable bag and is highly adjustable so keep adjusting until it fits just right. Also a plus that it looks great as well!
